Application Guidelines

Embracing the long-standing traditions of our predecessors, NCHS is committed to promoting a Healthy Community for all area citizens.

Since 1999, we have fulfilled this objective by financially supporting select activities of qualified not-for-profit organizations who share our vision, serving Benton Carroll, Clinton, Fountain, Montgomery, Tippecanoe, Warren, and White Counties.

We provide grants that will:

  • Advance technology and scientific development in health care.
  • Develop and promote community health education
  • Provide direct, individual social welfare and human service
  • Promote a Healthy Community
  • Improve the quality of life for all the communities’ people.

Organizations interested in initiating a grant request should begin by writing a letter of inquiry to North Central Health Services. Attn: Rita Smith, Program Director, P.O. Box 528 Lafayette, Indiana 47902. The letter of inquiry should include:

  • A concise description of the organization
  • A statement of the problem or need being addressed, and an explanation of the project
  • Estimated total cost, and the amount being requested from NCHS

Upon receipt of the letter of inquiry, initial review will determine whether or not the request meets the funding focus, and priorities of NCHS. You will be promptly notified in writing of the initial determination, and informed of application opportunities.
