"Hope Transit Program"
Area IV Agency on Aging

Grantmaking

NCHS is committed to addressing a wide range of health issues, and to enhancing the quality of life for individuals, families, and communities in our eight county Service Area. NCHS will support not-for-profit organizations and agencies that share our commitment to Health and Healthy Communities, primarily through grants for capital projects. NCHS prefers to fund projects that have a significant potential for positive impact on the community. NCHS encourages collaboration among agencies and organizations that efficiently utilize resources, and provide services that are not duplicative of existing programs.

The first step for an organization to apply for a grant is to submit a written letter of inquiry. This letter should include a concise description of the organization, a statement of the problem or need being addressed, and an explanation of the project, estimated total costs, and the amount being requested from NCHS. The letter should be addressed to:

North Central Health Services, Inc.
Attn: Rita Smith, Program Director
PO Box 528
Lafayette, In. 47902

Upon receipt of the letter of inquiry, initial review will determine whether or not the request meets the funding focus and priorities of NCHS. You will be promptly notified in writing of the initial determination, and informed of application opportunities.
